If you would like to become a new patient, please complete the following forms and return them to our office. Our physicians will then review your medical history to ensure that we can provide the level of care that you need. You should expect a call from our office staff within two business days of returning your registration forms.

Stayton Family Practice is excited to offer a new tool, the Patient Portal! It allows you to communicate with our staff more quickly, easily, and on your schedule. The Patient Portal delivers self-service capabilities for you to manage your healthcare at your fingertips. You do have to be a registered patient of Stayton Family Practice prior to using the Patient Portal.
What are your office hours of operation?
We are open Monday through Friday from 8:00am to 4:30pm.
How can I get a hold of my medical doctor?
We take pride in our doctor-patient relationships and schedule plenty of time for you to get all of your questions answered. If you need to get in touch with your medical doctor or physician assistant in between appointments, please contact our main office to leave a message or schedule an appointment.
Do you allow urgent or same day appointments?
We do try to set aside time for urgent visits. However, if you are having an emergency, please dial 9-1-1.
How do you notify your patients of lab or test results?
If your results are abnormal, our office will call you with your results and/or instructions. Normal results are usually mailed to the address we have on file. We can also send results to you through our secure patient portal, which can be the fastest way to receive your results. We are usually able to notify patients within 24 hours of receiving results.
Will I be able to get copies of my test results?
Yes, upon request we are able to provide you copies of your test results. Please notify the front desk staff if you would like to receive copies of your results. We can also send you copies of your test results through our secure patient portal.
How will I pay for my care?
If you have insurance, please bring in your card and we will either collect your co-pay or bill you through your insurance plan. If you are not insured, we are happy to make payment arrangements with you. We strive to have all aspects of our practice, including our professional fees, reflect our ethical and rational approach to your care.
What should I bring to my first appointment?
Please make sure that you arrive to the physician’s clinic at least 10 minutes before your scheduled appointment time and bring your insurance card and medications that you are currently taking.
Do I need a referral to make an appointment?
No, you do not need a referral to make an appointment with any of our providers.
What is your cancellation policy?
We prefer at least 2 hours notice for an appointment cancellation to be able to reuse your time slot. Please be sure to contact us as soon as possible if you will not be able to make your scheduled appointment.
Do I need to have my blood pressure checked regularly?
It is recommended that your blood pressure be checked every two years after the age of 21. We also recommend annual check-ups with your primary physician after the age of 50 and frequent check-ups if you have additional risk factors. The most common risk factors for high blood pressure include family history, being overweight and having a high sodium or poor diet.
How can I prevent a cold or flu?
Colds and the flus are commonly transmitted through the nose and mouth. To help prevent colds and flus:
Thoroughly wash your hands.
Avoid contact with contagious individuals.
After being in the company of someone with a cold or the flu, make sure not to touch your nose or mouth where the virus could enter.
Stay current on your yearly flu vaccinations.
Drink plenty of water to stay hydrated.
